The June 4 troop meeting is important for all Scouts going to Summer Camp and their parents. Topics to be covered include:
Summer Camp gear presentation for Scouts.
Summer Camp orientation for parents.
Youth Protection Training for Scouts (this is mandatory for all Summer Camp participants).
BSA Annual Medical Forms will be collected by Tammie Harrison (also mandatory for all Summer camp participants).
The June 11 Troop meeting is when swimming checks will be conducted. The location will be announced once it's known. Please note that swim checks are also mandatory for all Summer Camp participants.
Summer Camp will be here soon! Please follow these steps to submit your Annual Health and Medical Record:
Each scout and active leader/adult with the troop needs to submit a completed and signed Annual Health and Medical Record form, along with a front & back copy of your health insurance card. NOTE: This is a requirement for summer camp. Your son will not be allowed to participate in Summer Camp without having his forms submitted.
There are 4 parts to the form: Parts A, B, C and D.
Part A: Applies to all Scouts and is filled out and signed by a parent/guardian.
Part B: Applies to all Scouts and is filled out and signed by a parent/guardian.
Part C: Applies to all Scouts and is filled out and signed by your son’s doctor after a physical exam. Attach a front-and-back copy of your health insurance card to Part C.
Part D: Applies only to Scouts participating in a High Adventure Base camp, like Philmont, Seabase or Northern Tier. It is filled out by your son’s doctor after a physical exam.
Forms are valid for 12 calendar months up to the end of the applicable month. For example: if your son had a physical on May 9th 2012, then it is good through May 31st 2013. If he had a physical on June 9th 2012, then it is good through June 30th 2013.
Make a copy of all pages of the form you are submitting and keep it for your own records.
Submit the forms to Tammie Harrison at any Troop meeting.

is important for our troop to be scouting safely! In order to do that one of the things we need is to have annual medical forms on each scout and active leader/adult with the troop. These forms can be found on our troop web site if you click resources, then look under troop 364 information and click commonly used forms. You will see them under medical forms. Please do not use any other forms as they won't be accepted because they are not in compliance with the BSA.
